Chronic Disease Management Plans, Bulk Billed for Eligible Patients

Next Practice Newstead offers bulk billed Chronic Disease Management (CDM) plans for existing patients who meet the Medicare eligibility criteria. A CDM plan, also known as a GP Management Plan (GPMP) and Team Care Arrangement (TCA), is a structured, Medicare-funded care plan that outlines your health goals, the actions required to achieve them, and the healthcare providers involved in your care.

A GP Management Plan allows your GP to document your conditions, your treatment goals, and the services and referrals required to support your management. A Team Care Arrangement coordinates your care across a multidisciplinary team, allowing you to access Medicare rebates for up to five allied health visits per calendar year under your plan.

CDM plans are reviewed regularly to ensure they remain appropriate and effective as your health needs evolve. Please speak with your GP or our Patient Advocates to confirm your eligibility.

Multidisciplinary Care, Including In-House Physiotherapy

Depending on your circumstances, physiotherapy sessions may be partially or fully covered under your Team Care Arrangement and Medicare allied health rebates.
