What Is a Coordinated Veterans' Care Plan?

The Coordinated Veterans' Care (CVC) program is a DVA-funded initiative designed to support Gold Card and White Card holders who are living with one or more chronic health conditions. Under a CVC plan, your GP works with you and your broader healthcare team to develop a personalised, proactive care plan, with the aim of keeping you well and reducing the need for unplanned hospital admissions.

Who Is Eligible for DVA-Funded Care at Our Clinic?

DVA healthcare services at Next Practice Newstead are available to holders of a DVA Gold Card, who are entitled to treatment for all health conditions; holders of a DVA White Card, who are entitled to treatment for accepted service-related conditions; and veterans who may be eligible for specific DVA-funded programs such as the CVC program.
